A.  $30.00 Admission Fee (non-refundable)

B.  $20.00 Arkansas State Barber Board Issued Permit

C. Copy of your High School Diploma or Transcript, or G.E.D.

D. Original Social Security Card (copy will be made)

E.  Two (2) wallet size photos of yourself

F.  Copy of your Driver's License or State Issued ID Card

G.  Copy of your 2009 Federal Income Tax Return (If you filed) 1040, 1040A, or 1040EZ (If selected for verification)

     *Copy of your parent's 2009 Federal Income Tax Return (if they filed) 1040, 1040A, or 1040EZ   (If selected for verification)

     *Only if you are a Dependent Student =  You are under 24 years of age, and you have no children, and you are not married.
